Antiviral drug, nucleoside reverse transcriptase inhibitor. When entering the cell, it is metabolized to 5'-triphosphate that inhibits HIV reverse transcriptase which results in viral replication blocking. Lamivudine is active against zidovudine-resistant strains and slows down development of zidovudine resistance, when used in combination with the drug in treatment naive patients. It has, in vitro, a high therapeutic index that is greater than that of zidovudine (its cytotoxicity to peripheral blood lymphocytes, to lymphocyte and monocyte-macrophage cell lines, and to bone marrow progenitor cells is less than that of zidovudine). It does not interfere with cellular deoxynucleotide metabolism and has little effect on mitochondrial DNA content in intact cells. Lamivudine is highly active against hepatitis B virus (HBV) in all cell lines tested and in experimentally infected animals.
