Teriflunomide is an immunomodulatory drug with anti-inflammatory properties that selectively and reversibly inhibits the mitochondrial enzyme dihydroorotate dehydrogenase (DHO-DH), which is required for de novo synthesis of pyrimidine. Thus, teriflunomide blocks the proliferation of stimulated lymphocytes that require de novo pyrimidine synthesis. The exact mechanism by which teriflunomide exerts its therapeutic effect in multiple sclerosis is not fully understood, but it may involve a decrease in the number of activated lymphocytes in the central nervous system (CNS). It is possible that teriflunomide reduces the number of peripheral activated lymphocytes that may migrate to the CNS.
