Combination antiviral agent. Lamivudine and zidovudine are highly effective selective HIV-1 and HIV-2 inhibitors. Lamivudine is synergistic with zidovudine, inhibiting the replication of HIV. Both active ingredients of the combination drug are undergo successive metabolic activation by intracellular kinases to form their triphosphates. Lamivudine triphosphate and zidovudine triphosphate are substrates for and competitive inhibitors of HIV reverse transcriptase. However, the main antiviral activity of lamivudine and zidovudine is through incorporation of the monophosphate form into the viral DNA chain, resulting in chain termination. Lamivudine and zidovudine triphosphates show significantly less affinity for human cell DNA polymerases. Lamivudine and zidovudine combination therapy slows down emergence of zidovudine resistance in antiretroviral naive patients. Lamivudine in combination with zidovudine reduces viral load and increases CD4+ counts. Lamivudine and zidovudine combination therapy reduces the risk of disease progression and death.
