Linezolid, a synthetic antibacterial medicinal product, belongs to a new class of antimicrobial agents, the oxazolidinones, which are active in vitro against aerobic Gram-positive bacteria, some Gram-negative bacteria and anaerobic microorganisms. Linezolid selectively inhibits bacterial protein synthesis. By binding to bacterial ribosomes, it prevents the formation of a functional 70S initiation complex, which is an important component of the translation process in protein synthesis. The medicinal product is active in vitro and in vivo. Gram-positive aerobes: Enlerococcus faecium (including vancomycin-resistant strains), Staphylococcus aureus (including methicillin-resistant strains), Streptococcus agalactiae, Streptococcus pneumoniae (including multidrug-resistant strains), Streptococcus pyogenes. The medicinal product is active in vitro. Gram-positive aerobes: Enterococcus faecalis (including vancomycin-resistant strains), Enlerococcus faecium (vancomycin-susceptible strains), Staphylococcus epidermidis (including methicillin-resistant strains), Staphylococcus haemolyticus, Viridans group Streptococcus spp. Gram-negative aerobes: Pasteurella multocida. Microorganisms resistant to linezolid: Haemophilus influenza, Moraxella catarrhalis, Neisseria spp., Enterobacteriaceae spp., Pseudomonas spp. Resistance. The mechanism of action of linezolid differs from those of other classes of antimicrobial medicinal products (for example, aminoglycosides, beta-lactams, folic acid antagonists, glycopeptides, lincosamides, quinolones, rifamycins, streptogramins, tetracyclines and chloramphenicol); therefore, no cross-resistance exists between linezolid and these medicinal products. Linezolid is active against pathogenic microorganisms that are either susceptible or resistant to these medicinal products. Resistance to linezolid develops slowly through a multistep mutation in 23S ribosomal ribonucleic acid and occurs at a frequency below 1 × 10–⁹–1 × 10–¹¹.
